MUST HAVE 3-4 YEARS HOME HEALTH CARE EXPERIANCE.
Responsibilities:
  • Conduct comprehensive assessments of patients' medical history, current condition, and treatment needs
  • Develop and implement individualized care plans for patients based on their specific needs and goals
  • Coordinate and monitor patient care, ensuring that all necessary medical services and resources are provided
  • Collaborate with healthcare professionals, including doctors, nurses, and therapists, to ensure effective communication and coordination of care
  • Advocate for patients' rights and ensure that they receive appropriate medical treatment and support
  • Monitor and evaluate the effectiveness of treatment plans and make adjustments as necessary
  • Maintain accurate and up-to-date medical records for each patient
Skills:
  • Strong knowledge of anatomy, physiology, and medical terminology
  • Proficiency in using electronic medical record systems
