Pay:
$60,000-$80,000 annually. Pay is dependent on applicant's relevant experience. Duties Maintain resident medical records in accordance with facility policies, HIPAA requirements, and regulatory standards. Assemble, organize, and audit medical records for completeness and accuracy. Review documentation for required physician signatures, orders, assessments, and other regulatory requirements. Manage resident admissions, transfers, discharges, and record retention processes. Scan, upload, index, and file electronic and paper records as appropriate. Respond to requests for medical records while ensuring proper authorization and confidentiality. Track and process release-of-information requests. Assist with state surveys, audits, and regulatory reviews by providing requested documentation. Monitor record completion and follow up with physicians and department leaders regarding missing documentation. Maintain the facility's electronic medical record (EMR) system and assist staff with record-related questions. Collaborate with nursing, therapy, social services, admissions, and other departments to ensure accurate documentation. Support quality assurance and compliance initiatives related to medical record management. Protect resident privacy and maintain strict confidentiality of Protected Health Information (PHI).
Requirements:
Previous experience in medical records, health information management, or healthcare administration preferred. Experience in a skilled nursing, post-acute care, rehabilitation, or long-term care setting preferred. High school diploma or equivalent required. Associate degree in Health Information Management or related field preferred. Knowledge of HIPAA, medical terminology, and health information management practices. Understanding of federal and state regulations governing healthcare documentation. Strong organizational and time-management skills. Excellent attention to detail and accuracy. Proficiency with electronic medical record systems and Microsoft Office applications. Ability to handle confidential information professionally and ethically. Strong written and verbal communication skills. Ability to work independently while collaborating effectively with interdisciplinary teams.
