This is an opportunity for an Illinois Certified Recovery Support Specialist to serve as a member of an Assertive Community Treatment (ACT) team providing a very specialized model of treatment/service delivery. A multi-disciplinary team serves a small, defined caseload of seriously mentally ill (SMI) adults and becomes the single point of responsibility for that caseload. This is a unique treatment model in which the majority of mental health services are directly provided by the ACT team in the client's regular environment. Knowledge, skills and abilities :- Ability to use lived experience to assist people in recovery
- Ability to communicate effectively orally and in writing
- Able to use Microsoft Office, electronic health records and other reporting programs
- Willingness and ability to flex schedule as program needs change
Primary Duties :
- Provide direct service to ACT clients in their homes / places of residence
- Be familiar with the treatment plan for each client and assist with monitoring progress
- Work closely with the ACT Team Leader, Psychiatric Mental Health Nurse Practitioner, Registered Nurse, Case Manager, and Peer Specialist to coordinate activities and ensure each client receives the highest level of service
- Attend on-site a daily one-hour meeting, at least four days per week, with the ACT team
- Document services and submitting billing information in system of electronic health records on same day of service
- Respond to occasional after-hours or weekend phone calls and emergencies
- Know and work within federal and state rules, laws, and regulations. Ensure compliance with Joint Commission, SAMHSA, HFS, IDHS, and other requirements.

ACT Team Leader Requirements :
- Current Illinois certification as a Certified Recovery Support Specialist (CRSS)
- High School diploma, GED, or equivalent
- Computer literate
