MDS Director (Skilled Nursing Facility) - Job DescriptionPosition Summary The MDS Director is responsible for overseeing the Resident Assessment Instrument (RAI) process, including completion of Minimum Data Set (MDS) assessments, care planning, Medicare and Medicaid documentation, and regulatory compliance within the Skilled Nursing Facility (SNF). This role ensures accurate reimbursement, interdisciplinary coordination, quality outcomes, and adherence to federal and state regulations. The MDS Director collaborates with nursing leadership, therapy, social services, dietary, and other departments to support resident-centered care and operational excellence. Essential Duties and Responsibilities Coordinate and oversee the completion of all MDS assessments in accordance with CMS guidelines and regulatory timelines. Ensure accuracy of resident assessments, care area assessments (CAAs), and comprehensive care plans. Lead interdisciplinary care plan meetings and ensure individualized resident goals are addressed. Review clinical documentation to support coding accuracy and reimbursement optimization. Monitor resident status changes and ensure appropriate assessment scheduling. Regulatory Compliance Maintain compliance with all federal, state, and local regulations related to MDS, RAI, PDPM, and quality reporting. Ensure timely transmission of MDS data to CMS and state agencies. Prepare for and participate in regulatory surveys, audits, and inspections. Monitor Quality Measures (QMs) and Quality Reporting Program (QRP) indicators and implement improvement initiatives. Reimbursement & Financial Oversight Optimize Medicare and Medicaid reimbursement through accurate PDPM coding and documentation. Collaborate with billing, finance, and therapy departments to resolve reimbursement discrepancies. Conduct audits of documentation and coding practices to ensure compliance and maximize revenue integrity. Track Medicare utilization and insurance authorizations as needed. Leadership & Collaboration Serve as a resource and educator for nursing staff regarding documentation standards and regulatory requirements. Collaborate with the interdisciplinary team to ensure continuity and quality of care. Participate in Quality Assurance and Performance Improvement (QAPI) activities. Assist with orientation, training, and competency development related to MDS processes. Administrative Responsibilities Maintain organized and confidential resident assessment records. Develop and maintain assessment schedules and workflow processes. Generate reports and analyze trends related to reimbursement, compliance, and quality metrics. Perform other duties as assigned by facility leadership. Qualifications Current Registered Nurse (RN) license in the applicable state required; LVN/LPN may be considered based on state regulations and facility needs. Minimum of 2-3 years of MDS coordination experience in a Skilled Nursing Facility. Strong knowledge of CMS regulations, RAI process, PDPM, Medicare, Medicaid, and OBRA requirements. Experience with electronic medical records (EMR) and MDS software systems. Excellent organizational, analytical, communication, and leadership skills. Ability to manage multiple priorities and meet strict deadlines. Preferred Qualifications RAC-CT certification preferred. Previous supervisory or leadership experience in long-term care. Experience with Quality Measures and survey preparedness.
