Full Job Description Fundamental Objective:
As part of the Open Door Network's CalAIM Team, the Community Health Worker (CHW), under the supervision of the CalAIM Manager, is responsible for providing outreach, education, and support through systematic coordination of services and comprehensive care management
EDUCATION & EXPERIENCE

ESSENTIAL DUTIES AND RESPONSIBILITIES
Maintain accurate and up-to-date client records in accordance with agency policies and procedures. Responsible for providing outreach, education, and support through systematic coordination of services and comprehensive care management Send and receive external and internal referrals for participants needing CalAIM Services Act as a liaison between health services and the community to facilitate access to services and improve the quality and cultural competence of service delivery Effectively manage and maintain a caseload of assigned ECM members Enroll participants into the Coordinated Entry System and refer to appropriate community queues Input participants into Homeless Management Information System (HMIS) and other databases accurately and in a timely manner as needed Gather clinical information and assists with coordinating post-discharge services, including scheduling provider appointments, ensuring post-discharge referrals are received by the member, transportation to appointments is arranged, and members are aware of follow-care needs Reinforce health literacy and educational messages using appropriate tools and supplies within scope of practice Deliver services to vulnerable and high risk individuals particularly those hard to reach and or with social needs, in home and community based settings Provide basic education regarding chronic health conditions and health promotion a way that promotes understanding and self-management by the individual
ESSENTIAL POSITION REQUIREMENTS
Community Health Worker Certification 2 years free from any personal domestic violence charges Ability to operate telephone while taking notes Computer literate to create presentation outlines, maintain accurate records of statistical trends, etc. Have and maintain a valid California Driver's License, good driving record, reliable transportation, and automobile insurance; and must obey all traffic laws Understanding of confidentiality and privacy regulations related to sensitive client information. Strong interpersonal and communication skills to collaborate effectively with colleagues and external stakeholders
