Amergis Staffing is seeking
  • two Community Health Worker II (CHW II)
  • professionals for full-time, These positions support patients across multiple clinic sites and community events, serving low-income families and individuals with complex social and health needs.
  • Position Overview
  • The CHW II is a trusted community liaison who helps patients navigate medical care, behavioral health services, and essential social resources. This role requires compassion, strong communication skills, and comfort working in fast-paced, community-based environments.

  • Role Responsibilities
  • + Support patients across multiple sites and community events + Conduct outreach, engagement, care coordination, and case management + Link patients to internal and external resources (food, housing, transportation, financial assistance, etc.
) + Document clearly in EPIC and communicate effectively with providers + Provide telehealth support when needed (phone-based patient communication) + Advocate for patients and help navigate primary and specialty care
  • Required Qualifications
  • + •CHW Certification•Required•+ Compassion for underserved communities + Knowledge of local community resources + Strong communication and documentation skills + Comfortable using •Outlook, Teams, Windows, EPIC•, and other digital tools + Experience supporting populations facing homelessness, substance use, mental health challenges, HIV, or limited access to care (preferred) + Telehealth experience (preferred) + •Candidates who have previously worked at BACH will not be considered•Submission Requirements•Submittal Packet (ONLY these items):•+ Submittal Questionnaire + Resume •Do not include any additional documents.

  • Minimum Requirements:
