• What You'll Do
• + Connect patients with healthcare and community resources + Provide outreach, education, and care coordination support + Advocate for patients and help reduce barriers to care + Collaborate with medical providers and care teams + Document interactions and services in the medical record
• Minimum Qualifications
• + •Community Health Worker (CHW) Certification required•+ Strong communication and patient advocacy skills + Knowledge of community resources and social service programs + Passion for serving underserved and vulnerable populations Experience working with homelessness, behavioral health, substance use, housing resources, HIV services, or telehealth outreach is highly valued.

• Minimum Requirements:
