  • Community Health Center is seeking multiple Community Health Worker I/II candidates to support a variety of programs across Riverside County.
Community Health Workers (CHWs) serve as an important connection between the community and available health and social services, helping individuals, families, and vulnerable populations gain access to quality, ongoing care. As part of an interdisciplinary team, CHWs provide person-centered, community-based support through outreach efforts, wellness and prevention initiatives, health education, referrals, and guidance in navigating healthcare and social service systems. At the Community Health Worker I level, employees work under close supervision while developing foundational skills in areas such as client intake, outreach, and care coordination. Community Health Worker II employees perform duties with increased independence, delivering the full range of educational and support services while taking a more active role in population health management and client advocacy. Across both levels, CHWs are instrumental in reducing barriers to care, promoting healthier communities, and connecting clients with the resources and support needed to enhance their overall health and well-being.

For more information on RUHS-Public Health, please visit www.ruhealth.org/ruhs-public-health Build rapport with clients and gather accurate information to identify effective opportunities for intervention that reflect each client's values, attitudes, and beliefs. Provide clear, accessible health education and address barriers to physical and behavioral health services. Assist clients in navigating healthcare systems and public health programs through both in-person and phone-based support. Engage community members to raise awareness about violence prevention strategies. Build trusting relationships with individuals and families affected by asthma, offering emotional support, guidance, and practical assistance. Connect clients to essential community resources—such as housing, food, transportation, childcare, safety services, income assistance, and education—and make closed-loop referrals to address health-related social needs. Provide care management services for clients at highest risk of poor health outcomes, helping them access clinical and community services needed to carry out their care plans. Deliver transitional care services (TCS) for clients moving between care settings, maintaining consistent communication with clients and caregivers. Support clients' engagement with their primary care provider (PCP), promote wellness and prevention, assist with chronic disease management, and contribute to maternal and child health efforts. Participate in team collaborations, quality improvement initiatives, and program development activities. Ensure accurate, timely documentation in electronic health records and tracking systems in compliance with regulatory and accreditation standards.
    Community Health Worker/Promotore I Education:
    Graduation from high school, possession of a Certificate of Proficiency issued by the California State Board of Education, or attainment of a satisfactory score on a G.E.D. examination.
    Experience:
    Any combination of lived experience working with diverse populations in the community (e.g., women with infants and young children, homelessness, substance abuse, persons involved in the justice system, AIDS/HIV patients, older adults, and underserved populations) which provides an opportunity to acquire the knowledge and abilities listed. Normally, this includes one year of experience providing first-hand knowledge of the problems, needs, attitudes, and behavior patterns of the community as they relate to public health.
    Other Requirements:
    License:
    Possession of a valid California Driver's License may be required. Completion of a Department of Health Care Access and Information-approved CHW/Promotore training program and possession of a CHW/P/R certificate is required within 12 months of hire. Spanish bilingual Level I/II may be required
    Community Health Worker/Promotore II Education:
    Graduation from high school, possession of a Certificate of Proficiency issued by the California State Board of Education, or attainment of a satisfactory score on a G.E.D. examination.
    Experience:
    One year of experience as a Community Health Worker/Promotore I or equivalent non-County classification. OR Two years of lived experience equivalent to that of a Community Health Worker/Promotore I working with diverse populations in the community (e.g., women with infants and young children, homelessness, substance abuse, persons involved in the justice system, AIDS/HIV patients, older adults, and underserved populations) which would provide an opportunity to acquire the knowledge and abilities herein.
    Other Requirements:
    License:
    Possession of a valid California Driver's License may be required. Completion of a Department of Health Care Access and Information-approved CHW/Promotore training program is required. Must possess and maintain a CHW/P/R certificate. Some positions may require additional specialized certification. Spanish bilingual level I/II may be required.
    Physical Requirements:
    • Ability to stand for extended periods, with or without reasonable accommodation.
    • Ability to load and unload items weighing up to 20 pounds unassisted, with or without reasonable accommodation.
