Full Job Description Job Summary:
Join our team as a Community Health Worker (CHW) and make a meaningful difference in the lives of justice-involved individuals and other vulnerable populations. As a CHW, you will play a vital role in reconnecting incarcerated individuals transitioning back into the community with essential community resources, addressing social determinants of health, and empowering them to achieve their health and wellness objectives. We're seeking a compassionate, experienced professional who shares our passion for delivering person-centered care and promoting health equity.
Responsibilities:
1. Develop and maintain relationships with JI clients, providing support and guidance 90 days pre-release throughout their reentry process. 2. In-reach-to-clients who are incarcerated to conduct assessments to identify needs and develop personalized plans from pre- to post-release. 3. Connect clients with community resources, including healthcare, housing, employment, and social services. 4. Provide education and support on health and wellness topics, such as substance abuse, mental health, chronic disease management, and self-care. 5. Facilitate access to community resources, referrals, and workshops on topics relevant to JI clients' needs. 6. Collaborate with healthcare providers, social services, and justice system partners to ensure comprehensive support. 7. Coordinate care transitions and communicate with healthcare providers. 8. Maintain accurate records and reports, including client progress and outcomes. 9. Provide education and support on chronic disease management, healthy behaviors, and self-care. 10. Conduct regular home visits and phone follow-ups to monitor progress. 11. Document patient interactions and care plans in electronic health records within 24 hours and close all encounters. 12. Collaborate with interdisciplinary teams to address patient needs and improve health outcomes. 13. Identify and address social determinants of health and health disparities. 14. Provide cultural competency and linguistically appropriate services. 15. Maintain patient confidentiality and adhere to HIPAA guidelines. 16. CHW minimum productivity is expected to be 80% clinical (patient billable encounters) and 20% Non-productive (driving/documentation/outreach, etc.). 17. Conduct outreach to increase community awareness of the ECM program. 18. Attend regular staff meetings, training, and other meetings as requested.
NON-ESSENTIAL DUTIES AND RESPONSIBILITIES
Performs other duties as assigned. Ability to work independently and be self-directed and flexible. Ability to perform functions with minimal supervision. Ability to work at a high-volume level of accuracy.
Skills:
1. Strong communication, interpersonal, and cultural competency skills. 2. Ability to work independently and as part of a team. 3. Cultural humility and sensitivity. 4. Familiarity with community resources and services. 5. Basic computer skills and electronic health record (EHR) experience.
Qualifications:
Education Requirements:
1. A high school diploma/GED or equivalent is required. 2. Experience working with JI populations or similar vulnerable groups. 3. Community-based work or in-vivo work is needed with proper safety precautions. 4. Certification as a CHW (CCHW) or willingness to obtain. 5. CPI and CPR within 30 days of hire.
Preferred Qualifications:
1. An associate or bachelor's degree in psychology, social work, human services, or a related field is preferred. 2. Lived experience in the justice system or similar challenges. 3. Bilingual or multilingual skills. 4. Experience with motivational interviewing, trauma-informed care, or other evidence-based practices. 5. Knowledge of health and wellness topics, such as substance abuse, mental health, and chronic disease management.
Working Conditions:
1. CHW services are provided virtually or in person with locations in any setting, including correctional facilities, foster homes, residential facilities, group homes, outpatient clinics, hospitals, homes, or community settings. 2. Remote and Office environments for administration work. 3. Monday through Friday (8 hours). After hours and weekends for special events and outreach. 4. The caseload will be maintained between 40-45 Clients and may be higher to assist with overflow and/or proper CHW connection temporarily.
