Job Description
Project Description The Amelia Ann Adams Whole Life Center (AAAWLC) is a healing-centered, community-driven nonprofit dedicated to transforming systems of health, education, social justice, housing stability, and healing throughout San Joaquin County. Our mission is to cultivate change that moves individuals and families from surviving to thriving through culturally responsive, trauma-informed services rooted in equity, compassion, and community. The Community Health Worker (CHW) Case Manager provides person-centered care coordination and ongoing support to Medi-Cal members experiencing complex health, behavioral health, housing, and social challenges. Through a whole-person and trauma-informed approach, the Case Manager partners with participants to identify strengths, establish goals, navigate systems, and access the resources necessary to improve overall health, wellness, stability, and quality of life.
