Overview Join our dedicated team as a Community Health Worker and become a vital link in promoting health, wellness, and equitable access to healthcare services within diverse communities. In this role, you will empower individuals by providing education, support, and resources to improve their overall well-being. Your enthusiasm and commitment will help foster healthier communities through personalized outreach, advocacy, and collaboration with healthcare providers. Duties Conduct outreach and engagement with community members to assess their health needs and connect them with appropriate services Provide education on topics such as nutrition, medication adherence, addiction counseling, and preventive health measures Assist individuals with navigating healthcare systems, including Medicare enrollment and understanding medical terminology Support individuals with disabilities by offering tailored assistance and advocating for accessible healthcare options Collect data on community health trends and service utilization to inform program improvements and reporting Utilize motivational interviewing techniques to encourage positive behavioral changes and enhance patient motivation Collaborate with healthcare teams, social workers, and community organizations to coordinate care plans and resources Skills Strong knowledge of nursing principles, public health practices, and social work methodologies Experience working with individuals with disabilities or those facing complex health challenges Excellent communication skills for public speaking, teaching, and building rapport within diverse populations Ability to collect, analyze, and record data accurately for program evaluation purposes Familiarity with medical terminology, addiction counseling strategies, and Medicare policies Demonstrated ability to motivate others through motivational interviewing techniques Compassionate attitude combined with proactive problem-solving skills to address community needs effectively This position is an excellent opportunity for motivated individuals passionate about improving community health outcomes.
