PM Eastern FLSA Non-Exempt Bargaining Unit GRANTS Description Benefits Questions Description of Work Position Definition :
Provides outreach and education to Norwalk community members who may need information or assistance on health topics, Health Department programs and services, or community resources. Activities are intended to improve community health, increase access to information and services, and promote health equity in Norwalk.
General Duties:
Conducts community outreach activities, which may include attendance at health fairs and community events, information tables at community partner agencies, and development and delivery of information sessions or presentations. Assists with the planning, implementation, and evaluation of health promotion programs and initiatives. Assists with the development and distribution of educational materials as directed. Develops new partnerships and strengthens existing relationships with community partners. Coordinates with community members, community partners, and healthcare providers to provide support and education related to health services and access to community resources. Assists in gathering information to help identify potential barriers to accessing services and/or adopting health behaviors, possible solutions in specific communities, and potential gaps in support services in the community. Gathers and records data to assess program effectiveness and efficiency. Assists with Health Department programs and services. Assists other Health Department staff in service delivery, such as making appointments, registering clients, assisting clients with paperwork, verifying eligibility, processing clients in the Health Department electronic health record, assisting clinical staff members, etc. Provides staff coverage at the reception desk when needed. Answers phone calls, relays calls, and takes messages. Provides information and referral services to existing community services, both provided by the City and by other partners within the community. Communicates orally and in writing in both Spanish and English. Provides interpretation services for community outreach, health education, and clinical services
Additional Duties :
Attends health department and State Department of Public Health meetings as directed. Participates in quality improvement and evaluation activities and other duties as deemed necessary. Uses computer applications or other automated systems, such as spreadsheets, word processing, calendar, email, and electronic health record software in performing work assignments. May access protected health information (PHI) in accordance with departmental policies and procedures.
Supervised By :
Receives general supervision from the Assistant Director of Health, Community Health. Required Knowledge, Skills and Abilities Knowledge of the cultural, economic, and social factors affecting residents. Knowledge of Norwalk neighborhoods, available community resources, and partner agencies. Ability to interact with members of the public and establish interpersonal relationships with clients in a variety of settings. Ability to work effectively with partners and other staff members. Ability to show tolerance for differing points of view and beliefs. Ability to handle difficult situations courteously and tactfully. Ability to demonstrate sound judgment, resourcefulness, and initiative. Ability to use various computer applications and perform data entry. Ability to communicate effectively both orally and in writing. Minimum Qualifications High School Diploma or GED AND At least one (1) year of experience performing administrative and/or outreach activities. A Bachelor's degree in the human services, public health, social work, or other related field may substitute for the experience requirement. •Must be bilingual in Spanish and English.
Special Licenses/Certifications AND/OR Supplemental Information License or Certificate :
Valid CT Motor Vehicle Operator's License and insured, reliable transportation. Certification as a Community Health Worker (CHW) is preferred.
