Job Description
About us Citrus County Rural Health is a nonprofit community health center serving Citrus County and the surrounding area. We provide primary care to everyone regardless of insurance status or ability to pay. As a federally designated health center look-alike, we offer discounted care on a sliding scale based on household income. About the role Many of our patients arrive uninsured, underinsured, or unsure what they qualify for. You are the person who sits down with them and sorts it out. You will help patients apply for discounted care and public coverage, connect them to food, housing, transportation, and treatment resources in the community, and follow up to make sure the help actually landed. What you'll do Screen patients for eligibility and walk them through our sliding fee discount application, including income verification and annual renewals Assist patients with Florida Medicaid applications through the MyACCESS portal, plan selection, redeterminations, and appeals Help patients enroll in Health Insurance Marketplace coverage, Florida KidCare, and Medicare Savings Programs Refer and connect patients to community resources: the Citrus County Health Department, housing and utility assistance, food pantries and SNAP, substance use and behavioral health treatment, and transportation Help patients access prescription assistance and manufacturer patient assistance programs Document every encounter, referral, and outcome in the electronic health record, and follow up to confirm patients connected with the services they were referred to Build and maintain working relationships with local agencies and community partners Support reporting requirements related to patient demographics and enabling services What you need High school diploma or GED [or: Associate's degree in social work, human services, or a related field] [One to two] years in eligibility determination, social services, case management, patient navigation, medical front office, or a comparable role Comfort with computers, web portals, and electronic records Valid Florida driver's license and reliable transportation Ability to handle confidential information appropriately and maintain HIPAA compliance Nice to have Bilingual English and Spanish Direct experience with Florida Medicaid, MyACCESS, or Marketplace enrollment Certified Application Counselor (CAC) designation, or willingness to complete certification (we will train and certify you) Community Health Worker certification Familiarity with community resources in
