JOB SUMMARY
This position is responsible for assisting with the planning and delivery of the TEAM Dad program. The aim of the program is to enhance the health and well-being of children and families by supporting, encouraging, and promoting fathers roles in their childs lives. This position is responsible for providing direct services to support fathers involvement through case management, care coordination, education services, and fatherhood engagement initiatives.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Plan and conduct personal visits with fathers using evidence-based curriculum activities, methods, and materials Support expecting and parenting fathers by facilitating educational, economic, employment, mental health, housing, and healthcare needs as required Make referrals on behalf of fathers to needed resources and services in the community Maintain timely and accurate records of all personal visits, contacts, screenings, and resource linkages in the case management system, as set by program guidelines Maintain client confidentiality and integrity in keeping with agency policies, practices, professional standards, and program requirements Assist in the recruitment of fathers to participate in TEAM Dad program Participate in outreach activities to identify fathers, inform community of services, and build community collaborative relationships Collaborate with home visiting direct service providers and Connect representatives to facilitate enrollment of fathers into services Conduct home visits in assigned counties, and work evenings and weekends as necessary Participate in required trainings and staff meetings Stay current with relevant to maternal and child health issues, such as but not limited to racial disparities, infant mortality, prematurity, low birth weight, maternal infections, maternal stress, repeat teen births, smoking cessation during pregnancy, depression, optimal birth spacing, protective factors, father inclusion, social determinants of health, SUIDS, strategies for evidence-based interventions, substance use during pregnancy, substance exposed newborns, trauma informed care, and Life Course Perspective Perform all other duties as assigned and adhere to ongoing polices, program standards, and supervisory directives
QUALIFICATIONS
Education and Experience A Bachelors degree and one-year relevant work experience in community outreach and/or working with at risk fathers/families, social science, public health, health related filed such as nursing, health education, health panning, or health care administration preferred Minimum of two-years of college and three-years relevant work experience in community outreach and/or working with at risk fathers/families, social science, public health, health related filed such as nursing, health education, health panning, or health care administration High level of knowledge and experience with Microsoft and computer-based systems required/ preferred. Fatherhood and/or maternal and child health experience desirable Bilingual or Spanish speaking experience desirable Driving Requirements Possess a valid Florida Drivers License (required)- The employee must have a valid driver's license to drive their vehicle or a company vehicle to various worksite locations as required by the employer.• Must have a reliable vehicle Drive throughout assigned service area A safe driving record is required (Drivers License background check will be conducted prior to offer of employment and will be conducted annually for organizational insurance purposes) Other Requirements An FDLE background check and Level II Background Check will be required and may be conducted annually or as required at such times that they are warranted and/or that external funders require such a check. HIPAA and confidentiality training will be conducted upon hiring.
WORKING CONDITIONS AND PHYSICAL DEMANDS
Must be able to maintain stationary position for at least 50% of the time. Must be able to maintain a virtual work environment, if required. Must safeguard participant information and confidentiality. Must be able to occasionally move about inside the office to access file cabinets, copy machines, printers and front door. Must be able to consistently operate a computer and other office productivity machinery such as calculator, copy machine and printers. Must have the ability to communicate information and ideas so others will understand. Must have the ability to exchange accurate information with management and other employees. Must have the ability to observe details including inspecting, assessing and determining details/information. Must have the ability to focus and read forms and instructions for long periods of time. Must have the ability to bend, stretch and reach for items as needed. Must have the ability to walk distances and up stairs to complete home visits.
