PAB/Community Learning Service Worker- CARE Hawaii Inc.
Primary location:
Island Wide Responsibilities:
- Document and maintain Data collection for client's progress in regards to behaviors, incidents, health plans, home and community objectives.
- Transport client from home into the community setting safely
- Provide supportive assistance with client's in home and community daily living needs
- Contribute as a team member in treatment planning for clients
- Demonstrate effective communication with client,family members, and Service Supervisor.
- Work in a 1:1 setting in the home setting and in the community to execute responsibilities delegated by Service Supervisor.
- Meet once a month in the community setting with Service Supervisor for monthly supervision.
Requirements:
- High School Diploma or equivalent
- 6 months experience working with Developmental Disabilities
- Full time and Part time hours available
- Day, Mid , and Night shifts available
