Clinic:
Liza Medical is opening a new to-be-named primary care practice in Chelsea, MA, fully embedded within the La Colaborativa Survival Center. The practice offers a novel, community-based model of integrative care for vulnerable populations. We provide high-quality care fully integrated with La Colaborativa's robust and proprietary wraparound services that effectively mitigate social determinants of health, including targeted interventions for housing stability, economic mobility, food security, mental health, family stability, public benefits navigation, and the built environment. Designed to optimize patient/care team relationships and incorporating the most innovative technology, the practice is deliberately designed to build trust, dismantle barriers to healthcare access, deliver world class outcomes, and offer a host of supportive services targeting determinants of health that undermine community wellbeing.The Opportunity:
As a Promotora at our flagship practice in Chelsea, MA, you are an integral part of a physician-led primary care team that serves our community and supports their health needs. Your primary responsibilities focus on building trusting, supportive, and collaborative relationships with patients and their families. This work goes beyond what happens in an exam room and extends to patients' social needs and life experiences. You advocate for patients with a strong drive for service and a commitment to human connection. You balance the clinical requirements of excellent patient care with the social realities that influence health every day. You bring strong communication skills and enjoy collaborating with an integrated primary care team to support better healthcare outcomes for our patients and our community.Your daily responsibilities:
Capture relevant information about the patient's health and healthcare experience while rooming patients, taking vitals, administering point-of-care testing, and performing standard age- and condition-appropriate screening assessments Assist with the coordination of post-visit care by scheduling appointments with specialists, coordinating referrals, and sharing information with the patient's internal and external care team Collaborate with providers to monitor the health of a panel of patients and determine if they are up to date on preventive measures Participate in the daily operations of a primary care practice, such as answering incoming phone calls, responding to emails, assisting with front desk inquiries, and ensuring the general upkeep of the clinical space Assist in providing patient education on chronic disease management and coach patients using an action-planning model based on motivational interviewing techniques Provide anticipatory preventive guidance to families with children by establishing healing relationships with members and families Partner with staff of La Colaborativa as well as other service providers to make the navigation to different programs feel like a concierge service. A core component of our care delivery is leveraging human enabled technology with our partner Liza Health to ensure patient and doctor communication is seamless and reflects our values. Comfort with, and excitement about technology is required.Your experience:
At least 1 year of experience in healthcare as a Community Health Worker, LPN, Medical Assistant, Phlebotomist all welcome You either have the Community Health Worker certificate or are willing to become certified Strong written and verbal communication skills Experience working on collaborative and diverse teams Mission driven and invested in the health and well being of the Chelsea and broader La Colaborativa membership community Brings a positive, solution oriented attitude to their work Proficiency in computer technology such as typing, navigating the internet and using multiple software systems simultaneously. Personal interest in AI is a plus. Preference if you have successfully completed the La Colaborativa Community Health Worker Program Fluency in written and spoken Spanish and English Able to lift 20-30 lbs Fully vaccinated against COVID, Flu and relevant vaccines required byOSHA/CDC
You are authorized to work in the United States This is a full time in-person role. The final schedule is TBD.A possible schedule could be:

12pm-7pm The final schedule will be 40 hours per week at our practice in Chelsea, MA.
