Position Title:
Health and Elder Services Navigator Program:
New American Program Status:
Full-Time, Non-Exempt. Position is temporary through September 2026 and is contingent on continued grant funding.
Reports to:
Health Promotion Program Coordinator Jewish Family Service of Western MA is seeking a Health and Elder Services Navigator to join our multicultural New American program team. This staff member will serve as a client case manager and play a pivotal role in fostering connections and collaboration between JFSWM and the refugee and immigrant community. This position involves developing and implementing strategies to engage community members, addressing their needs, and promoting social integration.
Responsibilities:
1. Health Case Management and Navigational Support Conduct thorough intakes, eligibility screenings, and progress assessments to identify health-related needs and concerns for a caseload of clients. Complete necessary follow-up activities, supporting clients to schedule appointments and ensure they receive appropriate care. Assist clients in understanding medical terminology, navigating the healthcare system, interpreting health-related documents, and advocating for their health needs. Complete all necessary documentation of work with clients, including case notes, intake paperwork, and service plans. 2. Education and Group Programming Conduct outreach activities to raise awareness about preventive health measures and available healthcare resources to JFSWM clients. Provide education and training sessions on health-related topics to community members, such as basic health care navigation, chronic health condition management, nutrition education, etc. 3. Elder Adult Case Management Provide guidance to older adults on the path to citizenship. Support elders through cultural orientation and adjustment to life in the United States, ensuring they can navigate community resources. Connect clients with community resources that promote health, safety, and independence. Foster opportunities for social integration and engagement within the community to reduce isolation.
Qualifications:
Bachelor's degree in social work, public health, human services, or a related field. Experience working with refugee populations is highly desirable. Knowledge of refugee health issues, commitment to cultural sensitivity, and experience working within diverse communities. Demonstrated ability to work collaboratively and independently, to plan and organize work, ask relevant questions, and follow tasks through completion. Knowledge of local resources, social services, and agencies in Western Massachusetts, specifically those serving immigrant populations. Fluency in English and another relevant language is preferred (Swahili, Haitian Creole, or Dari/Pashto preferred) Previous experience in community engagement, preferably within refugee or newcomer communities or similar contexts. Demonstrated ability to plan and organize community events and initiatives. A valid driver's license, with an ability and openness to using personal and agency vehicles. A driving history check will be performed for the final candidate.
