Job Description
Job Requirements The Population Health Specialist will serve as the primary point of contact for participants in the Paradigm Grant program, a systems-based initiative that provides transitional housing for individuals experiencing homelessness. This role supports participants in developing individual goals, conducts screenings and assessments, and connects individuals with resources to support independent living Responsibilities Plans, implements, and monitors population health programs that support the mission, values, goals, and objectives of the system and the local site as directed by manager or director. Assists with writing and/or purchasing collateral material to support population health initiatives. Represents local site and/or UMMS to local community partners to build relationships and collaborate on programing. Works on planning and development of projects with a variety of internal and external partners. Assists in the development and implementation of population health programs for targeted populations. Perform all other duties as assigned. Work Experience Education and Experience High school diploma or GED is required. One (1) to Two (2) years of relevant experience is required. Knowledge, Skills and Ability Ability to manage, develop, plan, and deliver projects with minimal direction while meeting deadlines. Working knowledge of computer applications with experience in word processing and spreadsheet software such as Microsoft Word, PowerPoint, Excel, and CBISA. Excellent communication, interpersonal, and organizational skills. Ability to work effectively with the public, community partners, vendors, and professional staff. Cultural competence to understand the various backgrounds of the individuals served. Ability to organize and analyze data on a computer.
