Position Summary MAC, Inc. is seeking a compassionate and dedicated Dementia-Capable Community Health Worker (CCHW) to provide education, outreach, screening, care coordination, and support services to individuals living with dementia and their caregivers. This position serves as a critical connection between clients, healthcare providers, and community resources while promoting dementia awareness, caregiver well-being, early detection, and access to services throughout the community. This is 3-year contract position. Essential Duties and Responsibilities Caregiver Support Provide education, guidance, and emotional support to caregivers of individuals living with dementia. Conduct home visits, community visits, virtual appointments, and telephone contacts to assess caregiver needs and support systems. Connect caregivers with respite services, support groups, financial assistance programs, and other community resources. Facilitate educational workshops on dementia progression, behavior management, safety planning, and caregiver self-care. Assist families with navigating healthcare systems, insurance benefits, and long-term care planning. Advocate for clients and caregivers when coordinating services with healthcare providers and community organizations. Screening and Assessment Conduct Social Determinants of Health (SDOH) assessments. Screen caregivers for depression, social isolation, and medication management concerns. Administer the Alzheimer's Association AD-8 Dementia Screening Interview. Identify individuals at risk for cognitive impairment and facilitate appropriate referrals. Document assessment findings accurately and timely. Verify service connections and referrals through approved documentation systems. Follow up with clients and caregivers to support ongoing care coordination and service engagement. Community Outreach and Education Develop and deliver educational programs focused on dementia awareness, brain health, risk reduction, and early detection. Participate in health fairs, outreach events, screenings, and community presentations. Establish and maintain partnerships with healthcare organizations, senior centers, faith-based organizations, and community stakeholders. Distribute culturally responsive educational materials and resources throughout the service area. Care Coordination Coordinate services among clients, caregivers, healthcare providers, and community agencies. Assist with care planning and implementation of support services. Monitor client progress and service utilization. Maintain accurate and confidential records. Participate in interdisciplinary meetings, case conferences, and program activities. Required Qualifications High school diploma or equivalent. Must be a Certified Community Health Worker (CCHW) in the State of Maryland and maintain certification in good standing. Current CPR Certification. Valid driver's license and reliable transportation. Minimum two (2) years of experience working with older adults, individuals with chronic conditions, or in community health settings. Ability to travel throughout the Lower Eastern Shore service area. Preferred Qualifications Associate's degree or Bachelor's degree in a health-related field. Experience in dementia care, aging services, caregiver support, or related programs. Experience conducting screenings, assessments, and community outreach activities. Bilingual language skills. Experience with electronic health records, data entry systems, and case management software.
