This vital role provides outreach, resource navigation, education and advocacy to help individuals and families access health, social, and basic needs services. This role also serves as a health navigator for individuals and families in Ingham, Clinton, and Eaton counties, with a focus on reaching and supporting residents in rural communities.communities.
Qualificiations:
Minimum Education Required:
bachelor's degree in human services, public health, social work, psychology, or a related field preferred Experience working with children, youth, adults, and families in a community-based, health, or human services setting Knowledge of community resources, public benefits, and family support services Strong communication, organizational, and interpersonal skills Ability to work independently and as part of a team Valid driver's license and reliable transportation may be required Ability to pass required background checks and pre-employment screenings Responsibilities Provide direct outreach, engagement, and support to individuals and families Serve as a health navigator for clients in Ingham, Clinton, and Eaton counties, including rural communities, by helping them access health care, community supports, and social services- Assist clients with navigation of community resources, public benefits, and health and social service systems Connect families to food, clothing, shelter, transportation, parenting support, and other basic needs resources Provide education and support related to health promotion, wellness, family stability, and self-advocacy Maintain accurate documentation of client contacts, referrals, services, and follow-up activities Develop and maintain knowledge of community resources and share updates with staff and clients Collaborate with agency staff and community partners to coordinate referrals and support service delivery Participate in outreach activities, workshops, resource events, and family engagement initiatives Support reporting requirements, data tracking, and quality improvement efforts Assist with planning and implementation of community events, resource drives, and family-centered programming Maintain confidentiality, professionalism, and compliance with agency policies and procedures
Physical Requirements:
Requires the ability to exert light physical effort in sedentary to light work. Some lifting, carrying, pushing and/or pulling of objects and materials of light weight (5-10 pounds). Requires travel throughout Ingham, Clinton, and Eaton counties, including rural areas, for outreach, client support, and community engagement activities

$40,000.00 per year
