Job Description
Make a Difference in Your Community Looking for meaningful work where you can support people in their own homes and help them access the care and resources they need? In this 37.5 hour role per week, you will support adults with chronic health conditions, disabilities, and health-related social needs by providing care management and planning, health coaching, social care navigation, follow-up support, and connections to community resources. CHW certification is not required at hire. We will support the selected candidate through required Community Health Worker training and certification, making this a strong opportunity for someone with related experience who is ready to grow in community-based healthcare. Hybrid Work Eligibility This position is not immediately eligible for remote work. Employees are expected to complete onboarding and training, demonstrate proficiency with required systems and job responsibilities, and consistently perform work independently before being considered for a hybrid schedule. Hybrid eligibility typically occurs after a minimum of six (6) months of employment and only after independent performance has been demonstrated. This role includes regular travel to participant homes and community locations based on participant and program needs. What You'll Do
- Provide Community Health Worker support to Integrated Care at Home participants and caregivers.
- Visit participants in their homes and provide follow-up support between care visits.
- Help participants identify needs and connect them to resources related to food, housing, transportation, finances, caregiver support, and other health-related social needs.
- Provide health coaching, encouragement, education, and practical support to help participants work toward their care goals. Assist with outreach, scheduling, documentation, and participant engagement activities. Who May Be Successful in This Role This position may be a great fit for individuals with experience or education in:
- Human services, social services, public health, or community outreach
- Working with older adults, adults with disabilities, caregivers, or individuals with chronic health conditions
- Healthcare support, medical office support, or patient-facing service roles Qualifications
- High School Diploma or GED required. College coursework or Associate's degree in Human Services, Social Work, Psychology, Public Health, Healthcare, or a related field preferred.
- One year of related experience required.
