Position Summary:
Provides health education and connects NHS patients and community members to needed resources, proactively engaging individuals and supporting them in accessing services. Brings a demonstrated commitment to improving individual and community health. Interacts in a culturally competent manner with individuals and groups from diverse backgrounds—including but not limited to socio-economic status, race and ethnicity, nationality, sexual orientation, gender identity, and religion—applying principles of equity and inclusion across all settings.
Participates in one or more health projects, completing assigned training, meeting deadlines, and delivering on project deliverables in a timely manner. Develops trusting relationships with community members and patients, creating an appropriate foundation to deliver education about sensitive topics. Provides outreach and education at community sites, agencies, and health fairs (e.g., distributing materials), building relationships within NHS' focus populations. Works with patients to support and monitor care: Assesses patients and family's unmet health and related social needs and provides resources. Monitors patient adherence to care plans and encourages patient engagement. Conducts reminder calls and other outreach for patients due for screenings, preventive services, physical exams, and other preventive care visits and procedures. Assists with scheduling appointments. Reviews and applies relevant program materials, training resources, and health literature to stay current on assigned topics. Delivers health education one-to-one, in group settings, and within individual patient care, on topics including but not limited to family planning, unintended pregnancy prevention, sexually transmitted infections, chronic disease, mental health, and opioid harm reduction (after receiving training). Prepares and delivers presentations to community audiences on topics they have developed knowledge in through training. Tracks and documents data for activities and patients, including entries in the electronic medical record. Maintains strict confidentiality of patient information in compliance with HIPAA and NHS policies. Uses computer daily—including email, word and pdf documents, spreadsheets, and electronic medical record—as needed to carry out essential job functions. Travels to multiple program sites or clinics throughout the day. Performs other duties as assigned.
Required Qualifications or Abilities:
Minimum of HS Diploma/GED. Ability to maintain strict confidentiality in compliance with HIPAA and NHS policies. Speaks, understands, reads, and writes English sufficiently to carry out all essential duties.
Helpful Qualifications:
Minimum of 80 hours of community or social service experience, paid or volunteer, including formal or lived experience in a caregiving or advocacy role. Six months or more of experience delivering health education or conducting community outreach, in-clinic or in-community, to diverse populations—or work in a similar area. Health education experience in chronic disease, reproductive health and/or family planning, chemical health, or other health issues important to community. Existing community connections within the focus populations of NHS, including familiarity with Nort/Northeast Minneapolis or Anoka County. Spanish, Ukrainian, or other language fluency (written and oral). Knowledge of medical terminology; insurance plans; appointment scheduling; Electronic Medical Records. Demonstrated experience conducting community outreach, with comfort in public speaking. Valid driver's license, insurance, and access to a vehicle. Community Health Worker Certification or Medical Assistant Certification. This position is grant-funded. Staff may be assigned to different projects, populations, or initiatives based on grant requirements, funding availability, and organizational priorities.
