Position Summary:
Provides comprehensive health education and connects NHS patients and community members to needed resources, proactively engaging individuals and supporting them in accessing services. Leads or co-leads community health programs and projects, bringing a demonstrated commitment to improving the health of individuals and the community. Interacts in a culturally competent manner with individuals and groups from diverse backgrounds, including but not limited to socio-economic status, race and ethnicity, nationality, sexual orientation, gender identity, and religion—applying principles of equity and inclusion, both in-clinic and in the community. Requires flexibility to work evenings and weekends, accommodated through a flexed schedule or overtime approved on a case-by-case basis. Assigned to a primary clinic location, with regular travel to other NHS clinics and community sites as needed. Duties and Responsibilities Leads or co-leads health projects including, but not limited to, serving as the key contact for project funder, tracking work plan progression, compiling data, completing reports, monitoring expenses, and collaborating with essential partners to meet project goals. Assess community needs and assists in developing resources or projects to address them. Delivers health education one-to-one or in group settings on topics including but not limited to family planning, unintended pregnancy prevention, sexually transmitted infections, chronic disease, mental health, and opioid harm reduction (after receiving training). Prepares and delivers presentations to community audiences on health topics and available resources. Develops expertise on specific health topics and resources and trains other staff, volunteers, and partners on topic and information. Participates in or leads external meetings, trainings, and task forces, sharing information and promoting NHS programming. Develops trusting relationships with community members and patients, creating an appropriate foundation to deliver education about sensitive topics. Interacts in a culturally competent manner with individuals and groups from diverse backgrounds, including but not limited to socio-economic status, race and ethnicity, nationality, sexual orientation, gender identity, and religion, both in-clinic and in the community. Works with patients to support and monitor care: Assesses patient's and family's unmet health and related social needs and provides resources. Monitors patient adherence to care plans, and encourages patient engagement. Conducts reminder calls and implements other outreach for patients due for screenings, preventive services, physical exams, and other preventive services. Assist with scheduling appointments. Maintains strict confidentiality of patient information in compliance with HIPAA and NHS policies. Provides outreach and education at community sites and agencies, health fairs and events. distribution of flyers, etc. Builds new partnerships under direction of supervisor. Tracks and documents data for activities and patients, including entries in the electronic medical record. Travels to multiple program sites or clinics throughout the day. Performs other duties as assigned.
Required Qualifications:
Minimum of HS Diploma/GED with 2+ years of community health or related experience, including demonstrated experience leading projects with multiple deliverables and timelines; OR Bachelor's degree in Public Health or related field; OR equivalent combination of education, lived experience, and community health work. Strong written communication skills, particularly for business-style emails and reports. Competency in Microsoft Office Suite, especially Excel, Word and PowerPoint and basic computer skills. Proficient in spoken and written English, sufficiently to carry out all essential duties.
Helpful Qualifications:
One year or more of experience delivering health education—e.g., chronic disease, reproductive health/family planning, chemical health—in clinic or community settings to diverse populations. Existing connections within NHS' focus populations, including familiarity with North/Northeast Minneapolis or Anoka County. Spanish, Ukrainian, or other language fluency (written and oral). Knowledge of medical terminology; insurance plans; appointment scheduling; Electronic Medical Records. Demonstrated experience conducting community outreach, with comfort in public speaking. Community Health Worker Certification or Medical Assistant Certification.
