Location:
Downtown•St. Louis, MO The Community Health Worker will work under the supervision of the Health Services Manager and Public Health Nursing Supervisor for the Communicable Disease Section. This position serves as a critical bridge between the active TB and latent TB nursing teams, supporting coordinated patient care across the Tuberculosis Clinic and Communicable Disease Section. The Community Health Worker will partner with nurses and other community outreach employees to provide case management for patients with Tuberculosis Infection and Disease in St. Louis.
Duties include:
conducting community-based education and outreach; coordinating specimen pick-ups; delivering medication to patients in community settings; recording directly observed therapy (DOT); assisting with phlebotomy (blood draws) and taking patient histories in the TB clinic; and completing accurate and timely documentation of all medications, case management activities, and contract-required records. The incumbent must maintain meticulous organization and thorough documentation in accordance with the Tuberculosis Outreach Contract that the Department of Health receives from the State of Missouri. Incumbents are expected to work with autonomy when outside of the office after the initial training period. Duties are performed according to determined instructions and prescribed standards and procedures. The incumbent will be expected to work within deadlines and pay close attention to details.
Essential Functions and Responsibilities:
Works closely with both active TB and latent TB nurses to provide seamless case management, education, and communication across the care team (50%). Understands and utilizes principles and methods of TB disease investigation. Provides directly observed therapy (DOT) for tuberculosis patients at schools, homes, and other community locations; accurately record all DOT encounters. Coordinates specimen pick-ups and medication delivery for TB patients in the community. Assists with phlebotomy (blood draws) and patient history intake in the TB clinic to support medical providers. Completes detailed, timely data entry on paper and in online systems, ensuring all documentation meets State of Missouri contract requirements. Assists TB nurses with testing and educational events at homeless shelters and other community venues in St. Louis City. Provides health education and services to families, schools, shelters, and other community groups as needed. Case manager for latent tuberculosis infection (LTBI) cases, conducting home and school visits, and assisting with cases of tuberculosis disease as needed. Ensures medications are ordered, delivered, and/or picked up for LTBI and active TB cases; coordinate specimen collection logistics. Provides health education and services to families, schools, and other locations such as homeless shelters. Reviews case information with nursing staff and medical providers; communicate updates and follow up as directed. Serves as the coordination link between active TB and latent TB nursing staff, ensuring consistent communication and handoff of patient information. Assists in identifying client needs and referring clients to appropriate community services. Maintains client records in paper and online databases; manages correspondence, forms, and health insurance provider information with a high level of accuracy and organization. Uses Google Workspace tools (Google Docs, Sheets, Drive) and other technology platforms for documentation, communication, and data management. Performs all other duties as assigned.

Physical Requirements:
Requires the ability to lift and pull objects up to twenty (20) pounds.
Sensory Requirements:
Requires the ability to recognize and identify similarities or differences between characteristics of shapes and sounds to clearly distinguish objects and interview clients. Qualifications Two years of related health care experience with duties such as: home health visits; blood drawing/phlebotomy; medication administration; appointment setting; monitoring health records, client case management and/or field supervision. OR an equivalent combination of education, training, and experience.
Preferences:
Knowledge of tuberculosis. Child and adult health education experience. Medical Assistant (MA) license or certification. Community outreach experience. Direct observed therapy (DOT) experience. Phlebotomy experience. Work with underserved populations.
License:
Must possess and maintain a valid driver's license while employed by the City of St. Louis. Must possess a valid driver's license at the time of filing application and be able to present it upon request.
