Additional Information About the Role Missouri Baptist Sullivan Hospital has an opportunity for a Community Health Worker. In this role you will coordinate transportation services for patients by assisting with scheduling, logistics, and access to resources and serve as a liaison to help individuals navigate transportation challenges and reduce social barriers that may impact access to care and program participation. This is a FT on-site opportunity, Monday-Friday. Overview Missouri Baptist Sullivan Hospital is a designated critical access hospital committed to providing quality health care service to residents in Crawford, Franklin and Washington counties. The hospital has enhanced and expanded the health care services available to the greater Sullivan community with its state-of-the-art emergency department, modern childbirth center, critical care unit, multiroom surgical suite, expanded cardiac rehab services, and a cancer center.
A 22,500
square-foot medical office building provides offices for primary care physicians as well as space for a variety of visiting medical and surgical specialists from the St. Louis area. Missouri Baptist Sullivan Hospital operates four rural health clinics providing quality health care to surrounding communities. In addition, the hospital operates a comprehensive Therapy and Rehabilitation department and an award-winning Wound Care Center with two hyperbaric chambers. Preferred Qualifications Role Purpose Community Health Worker provides preventive health improvement services by working closely with interdisciplinary care team, internal/external agencies to address social determinants of health to improve the overall health and well-being of the child and family. Responsibilities Work daily as a community liaison to identify gaps in care by assisting clients in accessing the necessary resources to remove barriers categorized as social determinants of health. Provide referrals for appropriate services as needed by assisting in assessments/screenings or conducting follow-up care with patients in person, via phone, home visits and/or visits to other settings where clients are accessible. Establish and maintain trusting relationships with clients on a daily basis to effectively work with an inter-disciplinary team, internal departments, and with community organizations. Document and track ongoing activities, care plans, and results in an effective manner while strictly adhering to the policies and procedures in place. Continuously communicate and expand knowledge of community resources, services and in the hospital or community based agencies the programs provided to reduce cultural and socio-economic barriers between the clients and institutions.
