The Rural Health Transformation Program Navigator serves as a frontline liaison between community residents and available healthcare and social service resources. This role supports individuals in navigating complex systems, addressing social determinants of health, and improving access to care across assigned counties. Navigators support implementation of Hub-approved community health initiatives and collaborate with local partners to improve community health outcomes.
Position Details:
Minimum Qualifications Associate degree required; Bachelor's preferred in Social Work, Public Health, Community Health, Human Services, or related field. Two years of experience in community outreach, case management, patient navigation, or social service delivery preferred. Strong interpersonal, communication, and problem-solving skills. Ability to work independently in community settings with limited supervision. Preferred Qualifications Experience working in rural or underserved communities. Knowledge of social determinants of health and community-based resources. Completion of a Community Health Worker training program (such as Partners in Care) preferred. Ability to complete approved Community Health Worker training during the first year of employment preferred. Why Mercy?
