Job Overview:
The Maternal Community Health Worker will primarily be maintaining a presence in the hospital and the community with patients to promote, motivate and improve healthy living. Our Maternal CHWs serve as a vital link between pregnant women, families, and healthcare providers. The program focuses on preventive healthcare, emphasizing access to essential services during pregnancy and postpartum.

Newark Beth Israel Medical Ctr- Newark, NJ Local travel required inclusive of local churches, homeless shelters, and members residence
Essential Functions:
Meet patients in a hospital and conduct needs assessment using Social Determinants of Health Tool Schedule weekly follow-up calls to patients Assist patients with social issues like homelessness, substance abuse, hunger, transportation, and many other social issues that are impacting the members' lives Assist patients with organizing their records, making follow-up appointments with Primary Care Physicians and/or Specialists Help patients access resources, including quality of care, childcare services, food, housing, transportation Maintain relationships with the
