  • Community Health (FQHC) A mission-driven community health organization in the Albuquerque area is seeking a Case Manager to support patients by coordinating care, connecting individuals to community resources, and helping improve overall health outcomes.
This role partners closely with medical, behavioral health, and social services teams to ensure patients receive comprehensive and accessible care. The Case Manager will advocate for patients and help them navigate the healthcare system while addressing social determinants of health such as housing, transportation, food access, and behavioral health support. Key Responsibilities Provide case management services including care coordination, referrals, and resource navigation Conduct patient assessments to identify medical, behavioral, and social support needs Connect patients with community resources such as housing assistance, transportation services, food programs, and behavioral health support Collaborate with clinical and behavioral health teams to develop and implement individualized care plans Maintain accurate documentation in the electronic health record (EHR) system and track patient progress Support patient engagement, education, and follow-up to improve health outcomes and continuity of care Requirements Certified Peer Support Worker (CPSW) preferred or Bachelor's degree preferred in Social Work, Public Health, Psychology, or a related field 2+ years of case management or care coordination experience, ideally in an FQHC, community health center, hospital, or social services setting Experience working with underserved or vulnerable populations strongly preferred Knowledge of community resources and social service programs in New Mexico is a plus Strong communication, organizational, and patient advocacy skills Experience with electronic health records (EHR) and care coordination tools preferred A bit about us:
