Responsibilities:
As a Community Health Worker supporting the Santa Fe NM area you will make a meaningful impact on the health of children and their families. This role is embedded in Presbyterian Santa Fe Medical Center, connecting patients to community resources, providing basic health education, supporting care navigation, and co-leading the Produce Rx program for perinatal and pediatric participants. Qualified candidates bring experience in community health or clinical settings, a commitment to community wellbeing, and comfort using Microsoft Office and learning an electronic medical record. This position works primarily in person at the clinic, with some outreach in Santa Fe and surrounding communities. It is part of a collaborative statewide Presbyterian Community Health team, with remote meetings and occasional travel for trainings. This role includes a $4/hour differential to support cost‑of‑living needs in the Santa Fe area.
Key responsibilities include:
Following up on clinical and community referrals for pregnant and postpartum people Navigating patients to appointments, resources, and Medicaid enrollment Leading the Española Produce Rx program, enrolling eligible participants and coordinating weekly food distribution Building community partnerships and attending events to identify individuals needing support accessing care Documenting referral outcomes in Excel and the EPIC electronic medical record Providing feedback on workflows and standard operating procedures Level I qualifications: High school plus specialized, short-term training in clinical, medical skills. Must be able to speak language of the population served.
