This position is designed for individuals who are passionate about improving health outcomes and enhancing access to healthcare services for diverse populations. The CHW will engage with community members to provide education, support, and resources, focusing on public health initiatives and the unique needs of individuals, including those with disabilities. Responsibilities Serve as a liaison between healthcare providers and community members to facilitate access to services. Educate individuals and families about health resources, preventive care, and healthy lifestyle choices. Conduct outreach activities to raise awareness of public health programs and initiatives. Utilize motivational interviewing techniques to empower clients in making informed health decisions. Assist clients in navigating the healthcare system, including Medicare and other public health programs. Provide support for individuals with disabilities, ensuring they receive appropriate care and resources. Maintain accurate records of client interactions and follow up on referrals as necessary. Collaborate with social workers, nurses, and other healthcare professionals to coordinate care for clients. Deliver public speaking presentations on health topics to community groups. Qualifications Experience or education in public health, social work, nursing, or a related field is preferred. Familiarity with medical terminology and healthcare systems, particularly Medicare. Strong communication skills with an emphasis on public speaking and community engagement. Ability to conduct educational sessions and workshops effectively. Experience working with individuals with disabilities is a plus. Knowledge of motivational interviewing techniques is beneficial. A commitment to promoting health equity and addressing social determinants of health. Must be self-motivated, organized, and able to work independently as well as part of a team. This role is essential for fostering healthier communities through education, advocacy, and support.
