POSITION SUMMARY
The YMCA of Ithaca & Tompkins County is seeking a dedicated and compassionate Mobile Screener/Navigator to support our participation in the New York State 1115 Medicaid Waiver Social Care Network (SCN). The Screener/Navigator will serve as the first point of contact for eligible Medicaid participants by screening and referring them to Health Related Social Needs (HRSN) services. This role is responsible for screening individuals for social needs, providing culturally competent guidance, and facilitating referrals to internal and external service providers using approved SCN platforms. This position will be highly mobile, using the Ithaca YMCA as a base of operations, but will frequently be using our mobile food pantry to interact with participants in the rural areas of the Tompkins County. Please note that this role is supported by grant funding. As the grant period concludes, the YMCA will assess organizational needs, sustainability, and individual performance to explore potential opportunities for continued employment.

ESSENTIAL FUNCTIONS
1. Conduct standardized in-person HRSN screenings. 2. Engage with referred Medicaid participants in a respectful, empathetic, and culturally sensitive manner. 3. Identify participants' needs related to housing, food access, transportation, utilities, and other social care domains. 4. Use SCN digital platforms such as Unite Us and FindHelp to document interactions, make referrals, and track participant progress. 5. Follow up with participants to confirm receipt of services and ensure satisfaction. 6. Maintain accurate, timely, and HIPAA-compliant documentation of all communications and actions taken. 7. Assist participants with navigation of community resources and provide health literacy education. 8. Collaborate with the Director of Community Health to ensure SCN service standards are met, and quality benchmarks are maintained. 9. Drive or Ride Along with Mobile Food Pantry to rural locations in Tompkins County. 10. Assist in Food Hub and Food Pantry operations as well as Mobile Food Pantry and Prescription Food Box programs. 11. Assist in the delivery of evidence-based health intervention programming to support those with chronic diseases throughout Tompkins County. 12. Table and represent the YMCA at community based events. 13. Perform other duties as assigned.
QUALIFICATIONS
  • High School Diploma or GED required, associate or bachelor's degree in human services, social work, public health, or a related field preferred.
  • At least one year of prior experience working with Medicaid populations or in community health, case management, or care navigation preferred.
  • Familiarity with regional Social Care Network, Health-Related Social Needs services, and the Medicaid 1115 Waiver is a plus.
  • Strong interpersonal, verbal, and written communication skills.
  • Proficient with digital tools and case management platforms (e.g., FindHelp, UniteUs, or other referral software).
  • Bilingual candidates are strongly encouraged to apply, specifically in Ukrainian, Russian, Mandarin, or Spanish.
  • Ability to work independently, maintain confidentiality, and manage sensitive health and personal information.
  • Clear NY State Driver's License

WORK ENVIRONMENT & PHYSICAL DEMANDS

The employee frequently is required to sit and reach, and must be able to move around the work environment. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust. The noise level in the work environment is usually moderate. Will need to wear personal protective equipment when required.
