Job Description

Job Summary The Community Healthcare Worker (CHW) serves as a liaison between healthcare providers and community members, ensuring that individuals have access to necessary health services and resources. The role involves educating, advocating, and assisting people in managing their health conditions, promoting wellness, and accessing care, particularly in underserved populations. CHWs play an essential role in addressing health disparities and supporting community well-being. Key Responsibilities Community Outreach and Education Engage with community members to raise awareness about health issues and promote wellness programs. Provide education on topics such as chronic disease management, mental health, nutrition, maternal health, preventive care, and vaccination. Conduct health screenings and assessments as directed by healthcare professionals. Distribute health-related information and resources to community members. Health Support and Advocacy Help clients navigate healthcare systems, including scheduling appointments and referring them to medical services. Assist individuals in understanding their health conditions, treatment options, and the importance of adherence to prescribed therapies. Advocate for individuals in the community to ensure they receive necessary services and support. Act as a resource for clients in understanding healthcare benefits, insurance, and financial assistance programs. Case Management Maintain regular contact with participants to track health progress, address concerns, and provide support in achieving health goals. Work collaboratively with medical professionals and social services to develop care plans tailored to individual needs. Assist in the coordination of home visits or transportation for medical appointments when necessary. Cultural Competence and Community Engagement Build trust and rapport with community members, especially those from diverse and underserved backgrounds. Understand and respect cultural, economic, and social factors that influence health and wellness. Serve as a cultural liaison to reduce barriers and stigma associated with healthcare in the community. Data Collection and Reporting Record and document health information, services provided, and participants interactions accurately and confidentially. Collect and analyze community health data to assess needs and track health outcomes. Provide reports on outreach activities, participants progress, and health trends to supervisors and healthcare providers. Collaboration and Teamwork Work closely with healthcare professionals, social workers, and other community organizations to ensure comprehensive care. Participate in team meetings, workshops, and training to enhance skills and stay informed on healthcare trends and best practices. Qualifications Education and Experience High school diploma or GED required; some positions may require a degree or certification in a healthcare-related field (e.g., Public Health, Nursing, Social Work). Previous experience working in community health, social services, or a related field is preferred. Completion of a certified Community Health Worker training program within 6 months of hire. Human Trafficking 101 class CPR Certification Skills and Competencies Excellent interpersonal and communication skills. Strong cultural sensitivity and understanding of diverse populations. Ability to work independently and as part of a team. Strong problem-solving and critical-thinking abilities. Knowledge of local health resources, public health programs, and community services. Basic computer skills for data entry and reporting. Physical Requirements Ability to travel within the community for home visits, meetings, and outreach activities. Ability to stand, walk, and sit for extended periods. Other Requirements A valid driver's license and reliable transportation (if required). Ability to work flexible hours, including evenings or weekends if needed. Pay:
$13.50 - $17.50 per hour
