POSITION SUMMARY
The Community Health Worker (CHW) is a frontline public health professional who serves as a trusted member of and/or has a close understanding of the community served. This role functions as a liaison between community members and health and social service systems to improve access to care, enhance service delivery, and support improved health outcomes—particularly for individuals at high risk for HIV and other disparate health conditions. The CHW plays a critical role in outreach, prevention education, care coordination, and case management, while ensuring culturally competent and community-informed service delivery.

ESSENTIAL JOB FUNCTIONS
Conduct outreach and provide prevention education to individuals at high risk for HIV and other health disparities. Assist clients with navigating healthcare systems and accessing medical, behavioral health, and social services. Coordinate care and monitor client progress, including tracking compliance with care plans and treatment goals Provide case management services, including follow-up through phone calls, home visits, and community-based visits. Support clients in setting and achieving personal health-related goals Advocate for clients to ensure medical needs, referrals, and services are appropriately addressed Build and maintain relationships with healthcare providers, community organizations, and social service agencies Participate in staff meetings, trainings, and ongoing professional development Travel to off-site and community locations as required; occasional evening and weekend work expected Complete all required training and documentation in support of role responsibilities
QUALIFICATIONS & REQUIREMENTS
Bachelor's degree in Public Health, Community Health, Social Sciences, or a related field (preferred) OR Minimum of two (2) years of experience as a Community Health Worker or in a similar role Demonstrated passion for community engagement and improving health outcomes Strong interpersonal skills with the ability to build trust within diverse communities

Knowledge of community resources and service systems Strong communication and relationship-building skills Basic computer and documentation skills Ability to maintain strict confidentiality and adhere to HIPAA standards Ability to follow established policies, procedures, and compliance requirements Strong organizational skills and attention to detail Ability to work independently while maintaining accountability for outcomes
WORK ENVIRONMENT
This position is based at Guiding Right, Inc.; however, work is primarily conducted in community settings. The role requires frequent interaction with clients in various environments, including homes and community-based locations.
SUPERVISION & REPORTING
This position reports to the CHW Lead within the Prevention Services Division.

Maintain strict confidentiality of all client and organizational information Comply with all applicable federal, state (Oklahoma), and local regulations Adhere to all HIPAA privacy and security standards
ADDITIONAL REQUIREMENTS
May be required to receive vaccinations as part of public health service delivery
