We seek an experienced Community Health Worker (CHW) to join our fast- growing team and help grow the mission and impact. Job Overview The Community Health Worker (CHW) serves as a liaison between community members and healthcare providers to improve access to healthcare services and promote wellness. This role focuses on outreach, education, and advocacy within the community to address health disparities and empower individuals to make informed decisions about their health. The Community Health Worker plays a vital role in delivering culturally competent care and fostering positive health outcomes. Primary Responsibilities Community Outreach and Education Conduct outreach activities to engage community members and promote health education and awareness. Educate individuals and families on topics such as preventive care, nutrition, chronic disease management, and healthy lifestyles. Distribute educational materials and resources to support health promotion efforts.

Health Assessment and Support:

Conduct assessments to identify community health needs and barriers to healthcare access. Provide guidance and support to individuals in navigating healthcare systems, scheduling appointments, and accessing necessary services. Advocate for clients and assist them in understanding their rights and options for healthcare services.

Health Promotion and Disease Prevention:

Develop and facilitate workshops, support groups, and community events focused on health promotion and disease prevention. Collaborate with local organizations, healthcare providers, and public health agencies to implement health initiatives and campaigns. Encourage participation in screenings, vaccinations, and other preventive health measures.

Case Management and Referrals:

Maintain accurate records of client interactions, assessments, and referrals to healthcare providers or social services. Assist individuals in connecting with resources for housing, food assistance, mental health support, and other social determinants of health. Follow up with clients to ensure they receive necessary care and support. Educate individuals and families on topics such as preventive care, nutrition, chronic disease management, and healthy lifestyles. Distribute educational materials and resources to support health promotion efforts. Skills Strong background in community health, social work, or public health program planning with experience working with individuals from underserved communities Demonstrate cultural sensitivity and respect for diverse beliefs, traditions, and practices within the community. Advocate for equitable access to healthcare services and resources, addressing social determinants of health that impact health outcomes. Collaborate with community leaders and stakeholders to address health disparities and promote health equity. Proficiency in Microsoft Office applications for documentation, reporting, and communication purposes Ability to conduct health education sessions using motivational interviewing and culturally sensitive approaches Familiarity with basic life support (BLS) certification and nutrition or addiction counseling is a plus Bilingual abilities are highly valued and are required to effectively serve diverse populations Expected hours: 30.0 per week
