Job Description

B&D Integrated Health Services is seeking a Tailored Care Manager Extender (CME) to support our Tailored Care Management team in delivering whole-person care to clients. Under the direction of the TCM Supervisor, you will help connect individuals to services that support their physical and mental health, while promoting independence and self-sufficiency.
Key Responsibilities:
Provide outreach, engagement, and follow-up to support client care. Assist with appointment scheduling, reminders, and transportation coordination. Support health promotion activities and share progress updates with the care team. Help track referrals and connect clients to community-based resources. Participate in case conferences and support unmet health-related needs assessments.
Qualifications:
High school diploma or equivalent. Certification as a Peer Support Specialist or Community Health Worker. Behavioral health experience and EHR system knowledge required. Strong communication, organization, and computer skills. Compassionate, team-oriented, and driven to support others.
