Charleston, South Carolina Human Services Clinical & Research Support Services Full Time University (UNIV) Job Description Summary In support of the Maternal Fetal Medicine and Diabetes joint high risk pregnancy clinic at MUSC, the Community Health Worker is a frontline public health professional who serves as a vital link between the SC MOMs program and the community. This role is split between active community outreach, virtual and direct in-clinic patient support, and cross-functional collaboration. The ideal candidate is a trusted advocate who can navigate social determinants of health, facilitate education, and ensure patients stay engaged with their clinical care. Strategic Planning:
Partner with SC MOMs leadership to develop and execute a comprehensive community events and outreach calendar.Engagement & Trust:
Actively engage with community members to build trust and foster a deep understanding of the SC MOMs program.Education:
Provide on-site, hands-on, and interactive training at local events; advocate for the program to increase enrollment.Strategic Outreach Coordination:
Lead the end-to-end organization, planning, and on-site coordination of SC MOMs representation at all approved community events.Direct Support:
Conduct home visits or designated community location as necessary to support patients. 35%•In-Clinic Support:
Clinical Presence:
Attend weekly in-person SC MOMs clinics and provide support to both virtual and telemedicine providers and staff.Patient Advocacy:
Conduct screenings for Social Determinants of Health (SDOH) and build rapport with patients to identify and solve barriers to care.Retention & Follow-up:
Proactively reach out to patients to confirm upcoming appointments and ancillary visits to ensure continuity of care.Health Promotion:
Educate patients on medication compliance, treatment plan adherence, and the importance of regular screenings.Resource Management:
Manage the inventory of perishable/non-perishable pantries and assemble "New Patient Welcome Kits." 30%•Collaboration & Coordination:
Strategic Partnerships:
Act as a liaison with the Diabetes Initiative of South Carolina to conduct outreach and screenings at designated gatherings.Stakeholder Relations:
Partner with SC MOMs sponsors to enhance and represent the program's overall mission.Care Coordination:
Collaborate with healthcare providers, social services, and community partners to facilitate a highly engaged approach to patient care.Team Integration:
Participate in team meetings and contribute to collaborative projects aimed at improving SC MOMs and state wide health outcomes.Reporting:
Maintain clear, effective communication with internal and external leadership regarding program status and milestones.Preferred Experience, Knowledge, Skills:
Exceptional verbal and written skills with the ability to present information comfortably to both individuals and large groups. Proven ability to identify community health barriers and develop creative, empathetic solutions. Demonstrated success in building trust with diverse populations and clinical teams. Strong project management skills for planning events and managing clinic inventory. Ability to travel locally for community events and conduct home visits including some weekends. Prior experience in maternal health or diabetes management, combined with a background in community health or social services. Familiarity with the South Carolina healthcare landscape and community resources. Experience with public health forums and presentations. Ability to thrive in a fast-paced, high-stress environment. Fast learner regarding new systems, tools, and clinical equipment. Proficient in basic computer applications and data entry. Occasional weekend attendance required.Additional Job Description Minimum Requirements:
A master's degree and three years professional experience in human services or social services programs.Physical Requirements:
