Job Description

Job Summary Works in the neighborhoods where patients reside connecting and linking patients and their families to resources and the care they need. Serves as a member of the care model team enabling population health in the community by building trust and vital relationships with residents of assigned medical neighborhood. Essential Functions All team members are expected to be knowledgeable and compliant with Prisma Health's purpose: Inspire health. Serve with compassion. Be the difference Develops strategic partnerships in the community to support community outreach and referrals. Establishes peer-to-peer relationship in order to function as a link between the community and the health system. Provides guidance and support for patients through home visits and support of care coordination programs. Conducts home visits to Community Health Worker (CHW) clients. Assists individuals within their community in managing their own health including promoting compliance with appointments, screenings, treatment and medications. Assists patients in navigating the health care system. Facilitates assistance or interventions for patients between care visits. Advocate for patients by staying current of community resources. Provides resources to patients to improve their health knowledge and self-sufficiency, help them understand their health condition(s) and develop strategies to improve their health and well-being. Serves as a liaison between patients and clinical care team. Reports back to their clinical liaison on encounters and the results of such encounters. Coordinates meetings for CHW's to communicate and share best practices. Coordinates meetings with clinical liaison. Performs other duties as assigned. Supervisory/Management Responsibility This is a non-management job that will report to a supervisor, manager, director, or executive. Minimum Requirements Education - High School diploma or equivalent OR post-high school diploma / highest degree earned Experience - One (1) year of community involvement preferred. In Lieu Of NA Required Certifications, Registrations, Licenses Must be able to complete training and obtain certification after hire.
