The Community Health Worker is responsible for carrying out the assigned initiatives of Lifedoc Health which includes using the CHW training to aid in the development and implementation of activities that help patients become healthier. Drawing on their lived experience, the Community Health Worker will build trust with patients, helping them feel comfortable receiving assistance and connecting them to the resources and support needed. What You'll Do Works with Operations and Quality department leadership to develop performance improvement programs that support Darsalud Management's mission Participates in special projects as assigned, such as the Social Determinants of Health, , School Based Health System, Care Coordination, Health Maintenance, Patient Education Etc. Assists in the planning, training, implementing, monitoring, analyzing and reporting of activities that are designed to improve patient health outcomes and that DSM in compliance with regulatory and contractual requirements. Assists in the planning, facilitation, and coordination of population health activities related to the pursuit of continuous improvement in the following: the delivery of patient care, achievement of clinical outcomes, operational efficiency, patient satisfaction, patient safety Participates in the development of Quality Work Plan and Work Plan Evaluation Participates in and facilitates internal and external QI audits. Performs other job functions as requested or assigned What We're Looking For High School Diploma required, higher education attained is a plus Volunteer experience related to healthcare or community outreach is a plus Bilingual (English/Spanish) strongly preferred Good interpersonal, customer service and active listening skills Ability to manage change and produce results in a fast-paced environment. Strong analytical, communication and presentation skills. Excellent computer skills and knowledge of Microsoft Office Suite (Word, Excel and PowerPoint) Valid Driver's License Must be detail-oriented and highly organized Knowledge and understanding of medical practices and organizational policies Must be able to maintain confidentiality at all time Physical Requirements Prolonged periods of sitting at a desk and working on a computer Speak clearly so listeners can understand. Use fingers to grasp, move, or assemble very small objects. Use their hands to handle, control, or feel objects, tools, or controls Repeat the same movements.
