Job Summary The Community Health Worker (CHW) is responsible for providing outstanding customer service. This role will function as a member of a multidisciplinary team that conducts patient/family support to help patients achieve good health practices and community outreach. The position will facilitate health improvement, health promotion, health program development and support, disease prevention activities, and collaboration with regional programs and community partners. The THV team recruits and serves low-income, first-time pregnant woman to support healthy pregnancy, births, early childhood. This position generally functions within a clinical environment which may include travel between cities within the immediate counties which UTHSCT serves. Position may require frequent travel between normal locations and other buildings located on campus as well as travel to patient homes. May be exposed to inclement weather conditions and irate employees, visitors, and/or vendors. Potential exposure to patients with agitation and aggression. Probability of after-hours and weekends for workshops and networking. Conduct and participate in outreach activities in the program's service counties. Attend programmatic and community engagement activities representing the Nurse-Family Partnership and Texas Home Visiting. Collaborate with NFP nurse home visitors on the care of enrolled families. Assess families for unmet needs. Assist patients in determining their potential eligibility for grants, programs, and community resources. Aid clients with completing forms and applications required to obtain necessary services or meet referral agency requirements. Assist clients in scheduling future appointments and linking them to providers and/or needed services. Provide families with referrals to external community resources (such as food pantries), as needed. Complete timely documentation and required reporting for all services provided in departmental, programmatic, and/or funder tracking tools. Assist clients with completing and submitting vital documentation for annual enrollment, re-enrollment, and special enrollment periods to accommodate their healthcare needs. Provide one-on-one education on disease prevention and self-management while conducting home visits as needed for follow-up, health education, and application assistance. Comply with Texas Home Visiting requirements by promoting practices that align with programmatic philosophy, goals, and standards of care. Maintain confidentiality and respect patients' rights and privacy. Sustain quality service by establishing and enforcing organizational standards. Contribute to team effort by accomplishing related results as needed. Identify and attend regional community health activities on behalf of Texas Home Visiting. Collaborate with THV and other collaborative partners to promote population health. Research and validate referrals and supportive services available for program families. Sort, locate, and document referral emails and faxes in referral tracking tools. Ensure programmatic referrals are documented and routed both timely and accurately. Maintain professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, benchmarking state-of-the-art practices, and participating in professional societies. Handle all phone calls, emails, and interactions in person cordially and professionally. Other related duties as assigned. Required Education/Experience Bilingual (English/Spanish) High School diploma or GED required. Two (2) years of health or clinical experience or equivalent health-related experience. May accept a combination of education and experience. Preferred Education/Experience Outpatient Clinic experience preferred. EMR experience. Current Texas Community Partner Program training by Health and Human Services is preferred or obtained within 3 months of hire. Current Health Insurance Marketplace training is preferred or obtained within 3 months of hire. Understanding and experience with community resources and working with a variety of community-based organizations. Experience with care coordination or care management. Required Certifications / Licenses Current Community Health Worker certification by the Texas Department of State Health Services or obtained within 3 months of hire. Possess and maintain current CHW certification. Possess and maintain current CPR certification. Preferred Certifications / Licenses CPR certification or obtained within 3 months of hire. Exceptional customer service skills both in person and over the phone.
