Mobile Health Community Health Worker (CHW) Noble Community Clinics | Wautoma, WI | Full-Time Passionate about helping people and making a difference? About Us Noble Community Clinics is a Federally Qualified Health Center serving communities for over 60 years. We provide high-quality, accessible healthcare while putting patients, employees, and communities first. About the Role As a Mobile Health Community Health Worker, you'll connect patients to care and community resources—helping address social determinants of health and improve overall well-being. You'll work closely with clinical teams and community partners to support patients through education, outreach, and care coordination. What You'll Do Provide health education and outreach in community settings (homes, schools, clinics, etc.) Connect patients to community resources and support services Assist with care coordination, referrals, and follow-ups Document services in electronic medical records Support telehealth visits and care team initiatives Advocate for patients and help navigate healthcare and insurance systems What We're Looking For High School diploma required; Associate degree preferred CHW certification or related experience a plus Strong communication and community engagement skills Valid driver's license and reliable transportation Experience with diverse or underserved populations preferred What You'll Receive Competitive pay + full benefits 20 days PTO + 8 paid holidays Medical, Dental, Vision & Pet Insurance 401(k) with up to 5% match Company-paid life & disability insurance Supportive, mission-driven team culture Apply today and help build healthier communities.
