Job Description

•Applications are accepted until there are sufficient numbers of qualified applications or qualified candidate has been identified. The position could possibly close at any time.•Under the supervision of the Communicable Disease Manager and the Disease Control Lead nurse conducts investigation of general communicable diseases. Provides services to identified at-risk populations by telephone. Provides consultation and education regarding communicable disease prevention and control. Encourages the promotion and maintenance of health and the prevention of communicable disease and disability. Investigates reported communicable disease cases by interviewing infected individuals by phone to determine the likely source of illness, close contacts, and the potential for outbreak. Implements isolation and quarantine recommendations per protocol and teaches measures to prevent further spread to contacts and the community, monitors symptom progression/resolution, provides case management, and referral to appropriate community resources. Facilitates follow-up testing where appropriate. Works closely with Contact Tracing team and refers close contacts for quarantine and monitoring. Maintains awareness of community. Uses professional skills to identify individuals and groups whose behaviors place them at risk for health problems. Advocates for individuals and populations both within the human services department and the community. As a representative of the human services department, serves on committees and community forums. Informs individuals in the community of current human services departments, services, concerns, and goals through group presentations, personal contacts, written materials, and through the media. Participates in the selection and development of written materials for use in client education. Provides health information to the public and makes referrals to appropriate agencies. Provides consultation and direct services to defined at-risk populations. Maintains up-to-date case records and reports documenting service delivery and caseload management.

Education:
Possession of a Bachelor's Degree in a Social Science or a health related field.
Experience:
One year of experience that provided familiarity with Community and Public Health Programs. Familiarity with clinical language and ability to read and understand a medical chart and labs. Highly detail oriented, able to prioritize activities when under tight deadlines, and possesses analytical skills. Ability to handle confidential information with discretion and professionalism. Strong technical skills including word processing, spreadsheet development and management, and database management, with an emphasis on proficiency in Microsoft Office Suite. OR Associate's Degree in health related field and two years of experience that provided familiarity with Community and Public Health Programs. Familiarity with clinical language and ability to read and understand a medical chart and labs. Highly detail oriented, able to prioritize activities when under tight deadlines, and possesses analytical skills. Ability to handle confidential information with discretion and professionalism. Strong technical skills including word processing, spreadsheet development and management, and database management, with an emphasis on proficiency in Microsoft Office Suite.
Other Requirements:
Possession of a valid Michigan driver's license. Demonstrates effective communication skills by engaging, interacting and working with all individuals to ensure clarity and understanding in all interactions. Working Conditions:
This position works in an indoor environment. There is no planned exposure to prominent lights, noises, odors, temperatures or weather conditions.
Physical Requirements:
This position requires the ability to sit, stand, walk, climb, twist, bend, stoop/crouch, squat, kneel, lift, carry, push, pull, reach, type, endure repetitive movements of the wrists, hands or fingers. This position's physical requirements require periodic stamina in climbing, twisting, bending, stooping/crouching, squatting, kneeling, lifting, carrying, pushing, pulling, reaching, typing and enduring repetitive movements of the wrists, hands or fingers. This position's physical requirements require regular stamina in sitting, standing, walking. This position performs sedentary work requiring a negligible amount of effort in the physical requirements above. This position performs heavy work requiring the ability to exert more than 50 pounds of force in the physical requirements above. This position primarily requires close visual acuity to perform tasks within arm's reach such as: viewing a computer screen, using measurement devices, inspecting and assembling parts, etc. This position requires the ability to communicate and respond to inquiries both in person and over the phone. This position requires the ability to operate a PC/laptop and to enter & retrieve information from a computer. This position requires the ability to handle varying and often high levels of stress.
