The ideal candidate will possess a strong foundation in nutrition science and dietetics, with experience in patient assessment, care planning, and clinical nutrition. As a vital member of our medical staff, the Dietitian will develop personalized nutrition plans, provide health coaching, and collaborate with multidisciplinary teams to promote optimal health outcomes across diverse patient populations, including those in acute care, long-term care, and geriatrics settings. Duties Conduct comprehensive patient assessments to evaluate nutritional status and identify dietary needs. Develop and implement individualized care plans that address specific health conditions and nutritional goals. Provide expert guidance on specialized nutrition therapies and weight management programs. Monitor patient progress and adjust nutrition interventions accordingly to ensure effective outcomes. Educate patients and their families on dietary practices, lifestyle modifications, and health coaching strategies. Collaborate with healthcare professionals to integrate nutrition into overall treatment plans for acute care, geriatrics, and long-term care patients. Maintain accurate documentation of assessments, care plans, and progress notes within electronic medical record (EMR) systems. Applicants who speak spanish highly preferred.

Pay:

$45.00 - $50.00 per hour Expected hours: 40.0 per week
