Job Description

Job Summary We are seeking enthusiastic, energetic, self-motivated, and responsible Registered Dietitian to join our outstanding team at North Bay Post Acute in Petaluma, CA. On-call, two week days per week. North Bay Post Acute is a 98-bed skilled nursing facility located in a great community in Petaluma, CA. Join our dynamic healthcare team as a Registered Dietitian, where you will play a vital role in promoting optimal nutrition and health outcomes for diverse patient populations. In this energetic and rewarding position, you will assess individual nutritional needs, develop personalized care plans, and provide expert guidance to support patients' long-term wellness. Your expertise will help improve quality of life, especially within acute care, geriatrics, and long-term care settings, ensuring each patient receives comprehensive nutritional support tailored to their unique circumstances. Job Description Conduct thorough patient assessments to evaluate nutritional status, including diagnostic evaluations and physiological considerations. Develop and implement individualized care plans focused on nutrition goals such as weight management, disease management, or recovery support. Provide expert guidance on specialized nutrition interventions, including tube feeding and dietary modifications for various medical conditions. Collaborate with multidisciplinary teams to coordinate care and ensure nutrition strategies align with overall treatment plans. Offer health coaching to empower patients in making sustainable lifestyle changes related to diet and nutrition. Supervise and mentor junior staff or interns within the dietary department to uphold high standards of practice. Utilize electronic medical records (EMR) systems efficiently for documentation, tracking progress, and communicating with healthcare providers. Qualifications Bachelor's degree or higher in Dietetics, Nutrition, or a related field; registered dietitian credential required. Proven experience in SNF or hospital settings with a focus on acute care, geriatrics, or long-term care environments preferred. Strong knowledge of physiology and the application of dietetics principles to diverse patient populations. Hands-on experience with dietary interventions such as tube feeding and managing care plans for complex medical conditions. Familiarity with EMR systems for accurate documentation and communication. Supervising experience or leadership roles within dietary departments are highly desirable. Excellent patient assessment skills combined with the ability to deliver clear, compassionate guidance on nutrition and weight loss strategies. Pay scale Starting between $50 - $55 based upon experience Schedule We would prefer a Monday and Wednesday schedule from 9am-5:30pm and are open to alternate two week days per week.
