The ideal candidate will be responsible for assessing patients' nutritional needs, developing personalized care plans, and providing dietary education to promote health and wellness. This role is essential in ensuring that patients receive optimal nutritional support, particularly in hospital and acute care settings, including geriatrics and tube feeding scenarios.
Shift:
PRN, a few hours a week Duties Conduct comprehensive nutritional assessments for patients, focusing on individual dietary needs and preferences. Develop and implement tailored care plans that address specific health conditions, including weight loss strategies. Collaborate with the dietary department to ensure the delivery of appropriate meals and nutrition services. Provide education to patients and their families regarding dietary choices, nutrition labels, and healthy eating habits. Monitor patient progress through diagnostic evaluations and adjust care plans as necessary. Stay updated on the latest research in nutrition science, particularly regarding physiology knowledge relevant to patient care. Participate in interdisciplinary team meetings to discuss patient care strategies and outcomes. Skills Strong knowledge of hospital nutrition practices and experience in acute care settings. Experience in substance abuse clients preferred but not required Ability to create effective care plans based on diagnostic evaluations. Excellent communication skills for educating patients and collaborating with healthcare professionals. Critical thinking skills to assess patient needs and adapt plans accordingly.
