Bonuses or other incentives (if applicable) are offered separately and paid pursuant to the relevant program schedule. All employment offers will consider such factors as overall experience, job-related qualifications, location, certifications/training, etc. Plan, develop, implement and monitor the nutrition plan of care for each resident Provide timely nutrition assessments, care planning, and complet Minimum Data Set (MDS) assessments as required he nutrition plan of care is accurately and clearly communicated to the interdisciplinary team, resident and families via verbal discussions and written communication. Develop nutrition programs and systems are routinely monitored as part of the performance improvement program Determine patients at nutrition risk via nutrition screening Provide nutrition education to patients/families Provide nutrition counseling and education to patients Assure proper nutritional care for patients by conducting nutrition assessment Provide medical nutrition therapy (nutrition assessment, nutrition screening, counseling, education, modified diet formulation, nutrition intervention) for in-patients and out-patients Provide advanced medical nutrition therapy (nutrition assessment, nutrition screening, counseling, education, modified diet formulation, nutrition intervention, nutrition support) for in-patients Provide medical nutrition therapy (nutrition assessment, nutrition screening, counseling, education, modified diet formulation, nutrition intervention) for in-patients and out-patients Provide nutrition services and medical nutrition therapy integrated with the patient's medical needs Develop nutrition education materials/tools for patients Develop and implement medical nutrition care plans Perform comprehensive nutrition assessments and documents nutrition interventions in patient medical records Assess patients for nutritional status Providing nutrition assessments and care planning for patients Evaluate nutritional needs and develop a nutrition care plan Providing medical nutrition therapy (nutrition assessment, nutrition support, patient intervention and education) to meet the nutrition care needs Participate as a member of the Food Service Management team directly managing patient feeding Provide recommendations for enteral/parenteral nutrition needs and orders nutritional supplements to meet patient's nutritional needs Current Registered Dietician license in the state of Maryland Bachelor's degree with a major in Food and Nutrition, Dietetics, or a related field At least 2 years' experience as a dietitian, preferred Knowledge of the Nutrition Care Process and ability to apply all steps of the process A strong desire to deliver care in an empathetic, compassionate way Previous experience working in a skilled nursing facility highly desired Superb organizational and time management skills Excellent interpersonal, written, and verbal communication skills Ability to counsel and educate others Decision making, problem solving and collaboration skills a plus Strong leadership skills and ability to work collaboratively with team members Equal Opportunity Employer FutureCare has a longstanding policy of providing a work environment that respects the dignity and worth of each individual and is free from all forms of employment discrimination, including harassment, because of race, color, sex, gender, pregnancy, age, religion, national origin, citizenship, marital status, sexual orientation, gender identity, gender expression, physical or mental disability, military or veteran status, or any other characteristic protected by law.
