About us Mission Directed Health Care, Inc. is a small business with three locations in Minneapolis, MN. We are professional, agile and innovative seeking a Dietitian who has experience working with geriatrics. The ideal candidate will have extensive medical knowledge and experience caring for ailing patients, especially those experiencing memory loss. This is a great opportunity for candidates who have a strong work ethic, enjoy working independently and in a team setting, and have education and experience in geriatrics.
Responsibilities include:
Assess residents for all nutritional needs shortly after admission and quarterly. Develop and evaluate regular and therapeutic diets, including texture of foods and liquids to meet the needs of the residents. Participates in MDS assessments and completion timely and attends care conferences, updating the care plan at least quarterly and more often as needed. Consult with residents as needed to assist them in following the plan of care. Monitors resident weights monthly and responds to unplanned weight losses and gains. Assesses need for resident referral to other services (i.e. , swallowing assessment, etc.) Reviews menus for adherence to standards for dietary guidelines. Develops competency skills for dietary staff. Develops and implements person centered education programs involving food and nutrition for all facility staff. Participates in Quality Assurance and Performance Improvement committee (QAPI) quarterly. Attends other meetings as requested by the Administrator. Prepares educational programs on food related topics for the residents when requested. Chairs the Food Committee in conjunction with the Food Service Director.
