As a Nutritionist in our Active Ageing Centres, you will play a key role in promoting preventive health and active ageing among seniors through nutrition education, dietary assessments, and personalised nutrition interventions. Working closely with an integrated healthcare team comprising medical doctors, TCM physicians, nurses, and allied health professionals, you will support seniors in maintaining optimal health, managing chronic conditions, and enhancing their overall quality of life. Key Responsibilities Health Promotion & Education Design and conduct nutrition and wellness programmes for seniors. Deliver talks and workshops on topics such as healthy eating, hydration, chronic disease prevention, food safety, weight management, and healthy ageing. Develop educational materials and resources for seniors and caregivers. Conduct healthy eating campaigns and cooking demonstrations. Support national health and wellness initiatives within the community. Nutrition Assessment & Intervention Conduct nutrition screening and assessments to identify risks such as malnutrition, frailty, sarcopenia, obesity, and nutrition-related conditions. Provide individual nutrition counselling and dietary recommendations. Develop and implement nutrition care plans for seniors recovering from illness, surgery, or unintended weight loss. Monitor clients' progress and adjust interventions as required. Support seniors with chronic conditions such as diabetes, hypertension, hyperlipidaemia, kidney disease, and osteoporosis. Integrated Care & Collaboration Collaborate with multidisciplinary healthcare professionals in case discussions and care planning. Work closely with the Integrated Health Team to provide personalised, holistic care for seniors. Facilitate referrals to relevant healthcare and support services when necessary. Community Outreach Provide nutrition services at Active Ageing Centres and community-based outreach locations. Participate in community engagement activities, workshops, and wellness events. Requirements Diploma or Bachelor's Degree in Nutrition, Dietetics, Food & Nutrition, or a related discipline. Registration with the Singapore Nutrition and Dietetics Association (SNDA) or relevant professional accreditation will be an advantage. Experience in nutrition counselling, preventive health programmes, or community-based nutrition services. Strong knowledge of healthy ageing, chronic disease management, malnutrition prevention, and health promotion. Experience working with seniors and multidisciplinary healthcare teams is preferred. Good communication, presentation, and interpersonal skills. Willingness to travel between community locations as required. Working Hours Monday to
Friday:
8.30am to 5.30pm Occasional evening or weekend work may be required for community programmes and workshops.
