Full-time Responsibilities & Duties:
Interim position to cover for 2-5 months Responsible for the overall nutritional care of patients, Responsible for planning, assessment, implementation and evaluation of nutritional care plan of patients. Responsible for documenting nutrition care plans in the patient medical record, according to SNF policies and procedures. Assist in menu modifications and other nutritional related functions. 1. Interviews patient and/or family for nutritional history. 2. Obtains patient food preferences. 3. Assesses patients and documents in patients' medical record,. 4. Recommends nutrition care plans to medical staff for review and consideration. 5. Recommends therapeutic diet modifications and/or supplemental feedings to physicians for patients. 6. Ensures accuracy of diet order received by checking against physician's order. 7. Calculates nutrient content of tube feeding, TPN/PPN, and dietary intakes (calorie counts) as ordered by physicians. 8. Provides nutritional guidance and counseling to patients and their families. Provides written material as needed and according to prescribed diet. 9. Participates in Project Improvement projects; assists in data collection and reporting results. 10. Reports any malfunctioning of equipment which may interfere with a safe and smooth operation. 11. Assists in overseeing food service workers in proper food handing and portion control. 12. Assists manager with changes to the patient's menu, including patient food preferences, the development of standardized recipes, and overseeing food service workers in proper food handing and portion control. 13. Performs related duties, as required. Qualifications Bachelor's Degree in Food, Nutrition or Dietetics, required. Registered Dietitian, required. Active member of the Academy of Nutrition and Dietetics, preferred.
