Position Overview The licensed Functional Nutritionist provides individualized, evidence-based nutrition care and Medical Nutrition Therapy (MNT) as an integral member of the TNT Wellness clinical team. This role works collaboratively with physicians and nurse practitioners to assess patients' nutritional needs, translate clinical and laboratory findings into actionable nutrition strategies, and support patients in achieving sustainable improvements in metabolic health, weight management, hormone health, and overall wellness. The ideal candidate is passionate about functional and preventive nutrition, enjoys building meaningful patient relationships, and can balance evidence-based clinical nutrition with a personalized, whole-person approach to care. Key Responsibilities Medical Nutrition Therapy & Clinical Nutrition Provide individualized Medical Nutrition Therapy (MNT) within professional scope of practice and applicable state regulations Conduct comprehensive nutrition assessments, including dietary intake, medical and nutrition history, lifestyle factors, anthropometrics, laboratory data, medications/supplements, and patient goals Develop individualized nutrition care plans based on clinical needs, diagnoses, laboratory findings, treatment goals, and patient preferences Provide evidence-based nutrition counseling for conditions and concerns including: Prediabetes and diabetes Insulin resistance and metabolic dysfunction Medical weight management Cardiometabolic health Gastrointestinal and digestive concerns Hormone-related nutrition concerns Nutrient deficiencies Inflammation and chronic disease risk Healthy aging and longevity Monitor patient progress and modify nutrition interventions based on clinical response, adherence, and evolving needs Document nutrition assessments, interventions, education, follow-up plans, and patient progress accurately and in a timely manner Coordinate nutrition care with the patient's medical treatment plan and communicate relevant clinical findings to the medical team Functional Nutrition & Personalized Wellness Apply functional nutrition principles alongside evidence-based Medical Nutrition Therapy Review relevant laboratory and functional testing results within professional scope and incorporate findings into individualized nutrition recommendations Educate patients about the relationships among nutrition, blood sugar regulation, hormones, gut health, inflammation, sleep, stress, movement, and overall health Develop practical nutrition, meal-planning, and lifestyle strategies that patients can realistically implement Provide appropriate nutrition and supplement recommendations within professional scope and TNT Wellness protocols Help patients understand the "why" behind their recommendations and empower them to take an active role in their health Medical Weight Management Provide nutrition counseling and MNT as part of TNT Wellness medical weight management programs Collaborate with medical providers to support patients receiving pharmacologic weight management treatment Help patients optimize protein intake, nutrient density, hydration, meal timing, strength-supportive nutrition, and sustainable eating behaviors Monitor nutritional adequacy and support healthy body composition and long-term weight maintenance Identify and communicate nutrition-related concerns or barriers to the medical team Hormone & Metabolic Health Provide individualized nutrition support for patients participating in hormone optimization programs Educate patients on nutrition and lifestyle factors that may influence metabolic and hormonal health Collaborate with medical providers to ensure nutrition recommendations complement the patient's overall treatment plan Support patients through different phases of TNT Wellness programs, including initial assessment, optimization, and maintenance Patient Education & Experience Build trusting, long-term relationships with patients through education, accountability, and personalized support Translate complex nutrition and health information into clear, practical recommendations Use motivational interviewing and behavior-change strategies to support sustainable lifestyle changes Provide a high-touch patient experience consistent with the TNT Wellness standard of care Follow up with patients to assess progress, address barriers, and reinforce individualized recommendations Care Coordination & Team Collaboration Collaborate closely with physicians, nurse practitioners, and other members of the TNT Wellness team Participate in multidisciplinary case discussions and patient care planning Communicate relevant patient updates, clinical concerns, and barriers to progress to the appropriate medical provider Recognize situations outside the nutritionist's scope of practice and appropriately escalate concerns to the medical team Maintain accurate and timely documentation within the EHR and other applicable systems Program Development & Practice Support Provide nutrition services within TNT Wellness programs, including: Hormone Optimization Medical Weight Management Metabolic Health Digestive Health Functional Nutrition Contribute to the development and refinement of nutrition protocols, patient education materials, meal plans, resources, and clinical workflows Support ongoing improvement of TNT Wellness programs and patient journeys Participate in community education, wellness initiatives, and other nutrition-related outreach opportunities as appropriate Qualifications Required Bachelor's or Master's degree in Nutrition, Dietetics, or a related field One of the following professional credentials: Certified Nutrition Specialist (CNS) credential with applicable state licensure such as LDN or LN in North Carolina; OR Registered Dietitian Nutritionist ( RDN ) with applicable state licensure, such as LDN in North Carolina Strong foundation in Medical Nutrition Therapy and evidence-based nutrition care Experience providing individualized nutrition counseling Excellent communication, documentation, organization, and interpersonal skills Ability to work independently while collaborating effectively within a multidisciplinary clinical team Preferred Experience in functional, integrative, or preventive nutrition Experience with medical weight management Experience with metabolic health, insulin resistance, prediabetes, or diabetes Experience working with hormone health or BHRT patients Functional nutrition or integrative health training Familiarity with functional testing such as
