This role involves providing comprehensive nutritional care and counseling to diverse patient populations, including those in long-term care, acute care, and geriatrics settings. The ideal candidate will possess strong clinical skills, expertise in dietetics, and a passion for improving patient health outcomes through tailored nutrition plans. This position offers an opportunity to work within a collaborative environment utilizing advanced EMR systems and contributing to patient recovery and wellness. Duties Conduct thorough patient assessments to develop individualized care plans that address specific nutritional needs. Provide expert guidance on nutrition, weight loss, and dietary management for various conditions including geriatrics, acute care, and long-term care. Collaborate with healthcare teams to coordinate care plans, monitor progress, and adjust interventions as needed. Utilize EMR systems efficiently for documentation, tracking progress, and communicating with other healthcare professionals. Educate patients on nutrition principles, dietary modifications, and the importance of adherence to prescribed plans. Stay current with advances in dietetics, physiology knowledge, and hospital protocols to ensure high-quality patient care. Skills Proven experience in hospital or healthcare settings with a focus on geriatrics, acute care, or long-term care environments. Strong knowledge of physiology, nutrition science, dietetics principles, and specialized feeding techniques such as tube feeding. Proficiency with EMR systems for accurate documentation and communication. Excellent patient assessment skills with the ability to develop effective care plans tailored to individual needs. Ability to provide effective health coaching that motivates behavioral change. Knowledge of diagnostic evaluation processes related to nutritional status assessment. Strong organizational skills with attention to detail in managing multiple patient cases simultaneously. This position is integral to delivering exceptional nutritional care that enhances patient recovery and quality of life in Houston's healthcare facilities.
